Theoretically, horny goat weed might increase the effects and side effects of CYP2B6 substrates.
In vitro, horny goat weed leaf extract inhibits CYP2B6. This effect has not been reported in humans.

Here's the thing to know about thiotepa: it's actually a kind of prodrug. Your liver uses an enzyme called CYP2B6 to convert thiotepa into TEPA, the form that does a lot of the cancer-fighting work. In lab dish studies, horny goat weed appears to block CYP2B6. If that same blocking happened in people, it could shift how thiotepa is activated and cleared, which in turn could change both how well it works and how much toxicity (like low blood counts, nausea, or mouth sores) shows up.

I want to be honest about the strength of this: it comes from test-tube research only and has never been shown in real patients. Still, thiotepa is serious chemotherapy with little room for error, so please don't combine it with this herb without first clearing it with your oncology team.

Theoretically, horny goat weed might increase the effects and side effects of CYP3A4 substrates.
In vitro, horny goat weed extract inhibits CYP3A4 and suppresses CYP3A4 mRNA expression. This effect has not been reported in humans.

Here's the gist with Horny Goat Weed and thiotepa. Your body uses an enzyme called CYP3A4 to help process thiotepa. In lab dish studies (not in real people), compounds in horny goat weed slowed this same enzyme down. If that happened in the body, thiotepa could build up a bit higher than expected, which might mean stronger effects and more side effects, things like nausea, low blood counts, mouth sores, or a higher risk of infection.

I want to be honest though: this is theoretical, based only on test-tube data, and it has never been shown in humans. Still, thiotepa is serious chemotherapy, so it's not something to mix casually. Please run any supplement past your oncologist or pharmacist first.

About Thiotepa Injection

What Is Thiotepa Injection Used For?

Brand names: Thioplex, Tepadina

Tepadina (thiotepa) injection is used for several purposes. It treats adenocarcinoma (a type of cancer) of the breast or ovary. It is also used to control malignant effusions — abnormal fluid buildup in body cavities caused by cancer spreading to those areas. For bladder cancer, Tepadina treats superficial papillary carcinoma of the urinary bladder.

Tepadina is also used in pediatric patients with class 3 beta-thalassemia (a severe inherited blood disorder). In those patients, it is given as part of a preparative regimen — a conditioning treatment to reduce the risk of graft rejection before an allogeneic stem cell transplant (a transplant using donor stem cells), alongside the chemotherapy drugs busulfan and cyclophosphamide.
