Here is the interaction we found between these two, with the evidence behind it.

Minor

ANTIDIABETES DRUGS

Why this applies to Exenatide

Exenatide falls under ANTIDIABETES DRUGS — the group Ilex Guayusa interacts with.

Theoretically, taking Ilex guayusa with antidiabetes drugs might increase the risk of hypoglycemia.
Animal research in a model of hyperglycemia suggests that Ilex guayusa may have antihyperglycemic effects, although it does not appear to lower glucose levels when compared with baseline. This effect has not been evaluated in humans.
Severity Minor Likelihood Unlikely Evidence D

Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the honest picture with guayusa and exenatide (Byetta or Bydureon): this is a very mild, mostly theoretical concern. In some animal studies, guayusa seemed to nudge blood sugar down a little, so on paper, pairing it with a blood-sugar-lowering medicine like exenatide might add up to a slightly higher chance of going too low (hypoglycemia). This hasn't actually been shown in people, though.

If your sugar did dip too low, you'd feel shaky, sweaty, dizzy, or unusually hungry. Watch for that, especially since guayusa's caffeine can cause similar jittery feelings and blur the picture. You don't need to avoid your guayusa tea, but keep an eye on your readings and check with your pharmacist or doctor before making any changes.

From our pharmacist’s overview on the Exenatide page

“Because exenatide (Byetta, Bydureon) exists to lower blood sugar, nearly every supplement on this list pulls at that same lever, and I treat that as a genuine warning rather than a footnote. Most entries, including everyday items like fenugreek, bitter melon, flaxseed, and ginseng, can drop glucose further and raise the odds of hypogly…”

About Ilex Guayusa

What is Ilex Guayusa?

  • Energy and alertness
  • Mental focus
  • Antioxidant support
  • Appetite control
  • Athletic and physical performance

Ilex guayusa is a caffeine-containing holly leaf from the Amazon, traditionally brewed as a tea for energy and alertness, much like yerba mate or green tea. There is limited human research confirming specific health benefits, and its main reliable effect comes from its caffeine content. Because it is a stimulant, use it cautiously and check with your pharmacist or doctor if you have heart concerns or are pregnant.
