Inhaled Insulin falls under ANTIDIABETES DRUGS — the group Ilex Guayusa interacts with.

Theoretically, taking Ilex guayusa with antidiabetes drugs might increase the risk of hypoglycemia.
Animal research in a model of hyperglycemia suggests that Ilex guayusa may have antihyperglycemic effects, although it does not appear to lower glucose levels when compared with baseline. This effect has not been evaluated in humans.

Here's the gist with guayusa tea and inhaled insulin (Exubera). Insulin's whole job is to lower your blood sugar, and there's a theory that guayusa might nudge sugar down a little too. If both pulled in the same direction, you could in theory dip too low, which is what we call hypoglycemia: shakiness, sweating, a racing heart, confusion, or feeling lightheaded.

I want to be honest about how thin this evidence is. It comes only from animal research, and even there guayusa didn't clearly lower sugar below baseline. There's no human data showing this actually happens. So I'd file this as a small, theoretical concern rather than something to worry about. Still, if you use insulin, check your blood sugar as usual and let your pharmacist or doctor know you're drinking guayusa.
