These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Ilex Guayusa interacts with whole categories of medication. Nicotine falls into 2 of those categories, so it can interact with Ilex Guayusa in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
STIMULANT DRUGS
1 of 2Why this applies to Nicotine
Nicotine falls under STIMULANT DRUGS — the group Ilex Guayusa interacts with.
Ilex guayusa contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: both guayusa and nicotine are stimulants. Guayusa's main active ingredient is caffeine, and nicotine (whether from a patch, gum, lozenge, or inhaler) revs up your body in a similar way. Stack them together and those stimulant effects can add up.
What might you notice? Things like a faster or pounding heartbeat, feeling jittery or anxious, trouble sleeping, an upset stomach, or a slightly higher blood pressure. This is more of a theoretical, common-sense overlap than a proven danger, and it's based on expert opinion rather than hard studies. A cup or two of guayusa tea probably isn't a big deal for most people, but stronger concentrated doses are more likely to push the jitters. If you have any heart concerns, are pregnant, or just feel wired, please check with your pharmacist or doctor before combining them.

NICOTINE
2 of 2Ilex guayusa contains caffeine. Concomitant use of caffeine and nicotine has been shown to have additive cardiovascular effects, including increased heart rate and blood pressure. Blood pressure was increased by 10.8/12.4 mmHg when the agents were used concomitantly.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to keep in mind: both guayusa and nicotine are stimulants, and they tend to push in the same direction. Guayusa's main active ingredient is caffeine, and nicotine (whether from a patch, gum, lozenge, inhaler, or spray) also revs up your cardiovascular system. Used together, they can add up.
In one study, combining caffeine and nicotine raised blood pressure by about 10.8/12.4 mmHg and bumped up heart rate. So you might notice a faster heartbeat, a jittery or wired feeling, or higher blood pressure. This isn't a deal-breaker for most folks, but if you have heart trouble or already run high blood pressure, it matters more. Chat with your pharmacist or doctor before mixing the two regularly.

From our pharmacist’s overview on the Nicotine page
“Nicotine itself is a stimulant, whether it comes from a Nicoderm CQ patch, Nicorette gum, or a Nicotrol inhaler, so treat this list with genuine care. Of the 64 supplements and herbs on file, 44 add more stimulation on top of the nicotine, which can push heart rate and blood pressure higher than either would alone.”

About Ilex Guayusa
What is Ilex Guayusa?
- Energy and alertness
- Mental focus
- Antioxidant support
- Appetite control
- Athletic and physical performance
Ilex guayusa is a caffeine-containing holly leaf from the Amazon, traditionally brewed as a tea for energy and alertness, much like yerba mate or green tea. There is limited human research confirming specific health benefits, and its main reliable effect comes from its caffeine content. Because it is a stimulant, use it cautiously and check with your pharmacist or doctor if you have heart concerns or are pregnant.
