Here is the interaction we found between these two, with the evidence behind it.
STIMULANT DRUGS
Why this applies to Phentermine
Phentermine falls under STIMULANT DRUGS — the group Ilex Guayusa interacts with.
Ilex guayusa contains caffeine. Due to the central nervous system (CNS) stimulant effects of caffeine, concomitant use with stimulant drugs can increase the risk of adverse effects.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the simple version: phentermine is itself a stimulant that revs up your nervous system, and guayusa brings caffeine to the table, which is another stimulant. Stack two stimulants together and their effects can pile up. You may notice a faster or pounding heartbeat, jitteriness, anxiety, trouble sleeping, or a bump in blood pressure. Phentermine alone can already cause these things, so adding caffeine just turns up the dial.
This concern is based mostly on expert reasoning rather than hard studies, so it isn't a sure thing. A small cup now and then may be fine for many people, but larger or daily amounts are more likely to matter. Please check with your pharmacist or doctor before mixing them, especially if you have any heart or blood pressure concerns.

This page covers Ilex Guayusa only. If you take other supplements — a multivitamin, a sleep aid, something for joints — the Phentermine page lists 61 supplements it has a documented interaction with, worst first, each one explained.
The most serious there right now: Activated Charcoal (major), Ephedra (major), Peyote (major), Rauvolfia Vomitoria (major) and Sida Cordifolia (major).
By severity: 5 major · 43 moderate · 13 minor.
From our pharmacist’s overview on the Phentermine page
“Ephedra is the pairing I most want anyone taking phentermine (Adipex-P) to know about. Phentermine is itself a stimulant appetite suppressant that raises heart rate and blood pressure, and ephedra piles the same cardiovascular strain on top of it.”

About Ilex Guayusa
What is Ilex Guayusa?
- Energy and alertness
- Mental focus
- Antioxidant support
- Appetite control
- Athletic and physical performance
Ilex guayusa is a caffeine-containing holly leaf from the Amazon, traditionally brewed as a tea for energy and alertness, much like yerba mate or green tea. There is limited human research confirming specific health benefits, and its main reliable effect comes from its caffeine content. Because it is a stimulant, use it cautiously and check with your pharmacist or doctor if you have heart concerns or are pregnant.
