Here is the interaction we found between these two, with the evidence behind it.
CIMETIDINE (Tagamet)
Ilex guayusa contains caffeine. Cimetidine can reduce caffeine clearance by 31% to 42%.
Well-controlled human studies have demonstrated the likely existence of this interaction.
Here's the simple version: cimetidine slows down how fast your body clears caffeine. Since guayusa tea is basically a caffeine source (a lot like yerba mate or green tea), that caffeine can hang around longer than usual. Studies show cimetidine can cut caffeine clearance by roughly a third to nearly half, so a normal cup or two may hit you harder.
What might you notice? The usual caffeine effects, just stronger or longer: jitteriness, a racing or pounding heart, trouble sleeping, restlessness, or an upset stomach. This won't change how well cimetidine treats your heartburn or ulcer, it just amplifies the caffeine. If you enjoy guayusa, you may simply want less of it. Please check with your pharmacist or doctor before changing anything, especially if you have heart concerns or are pregnant.

About Ilex Guayusa
What is Ilex Guayusa?
- Energy and alertness
- Mental focus
- Antioxidant support
- Appetite control
- Athletic and physical performance
Ilex guayusa is a caffeine-containing holly leaf from the Amazon, traditionally brewed as a tea for energy and alertness, much like yerba mate or green tea. There is limited human research confirming specific health benefits, and its main reliable effect comes from its caffeine content. Because it is a stimulant, use it cautiously and check with your pharmacist or doctor if you have heart concerns or are pregnant.
About Cimetidine

Brand names: Tagamet HB, Tagamet, Cimetidine Injection
Cimetidine lowers stomach acid. The over-the-counter tablets (sold as Acid Controller, Acid Reducer, Basic Care Acid Reducer, Equate Cimetidine, Tagamet and others) relieve and prevent heartburn linked to acid indigestion and sour stomach.
The prescription cimetidine tablets and cimetidine hydrochloride oral solution treat stomach and digestive tract conditions. These include active duodenal ulcer, benign gastric ulcer, erosive gastroesophageal reflux disease (GERD) and pathological hypersecretory conditions. They are also used to keep duodenal ulcers from coming back after healing.
