Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the thing to know with this pairing: it's actually your estropipate that affects the guayusa, not the other way around. Estrogen can slow down how your body clears caffeine, and since Ilex guayusa is basically a caffeine-rich tea, the caffeine can linger longer and hit harder than you'd expect.
In real life that might mean more jitteriness, a racing or pounding heart, trouble sleeping, or feeling anxious or wired. Estropipate itself won't become stronger or weaker from the guayusa, so this isn't about your menopause symptom relief changing. The evidence here is moderate, so you don't need to panic, but if you're noticing caffeine bothers you more than usual, ease back on the guayusa and chat with your pharmacist or doctor.
