Promethazine falls under PHENOTHIAZINES — the group Ilex Guayusa interacts with.
Ilex guayusa contains caffeine. Phenothiazines can reduce the metabolism of caffeine by inhibiting cytochrome P450 1A2 (CYP1A2).

Here's the thing to know about pairing guayusa tea with promethazine (Phenergan): the concern is mostly about the caffeine in guayusa, not the promethazine itself. Promethazine can mildly slow down a liver enzyme called CYP1A2, which is the one your body uses to clear caffeine. In theory, that means the caffeine could hang around a bit longer and build up, so you might feel more of its effects, things like jitteriness, a racing heart, trouble sleeping, or restlessness.
