ANTIDIABETES DRUGS
Why this applies to Semaglutide
Semaglutide falls under ANTIDIABETES DRUGS — the group Indian Long Pepper interacts with.
Animal research shows that piperine, a constituent of Indian long pepper, can reduce blood glucose levels. Monitor blood glucose levels closely. Dose adjustments might be necessary.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with semaglutide (Ozempic, Wegovy, Rybelsus) and Indian long pepper. Both of these can lower blood sugar, so taking them together might push your sugar down a little more than semaglutide does on its own. The concern is mostly theoretical, coming from animal studies on piperine, the pepper's active compound, not from solid human research. Semaglutide itself rarely causes low blood sugar unless you also take insulin or a sulfonylurea like glipizide.
So this isn't a reason to panic. Watch for signs of low sugar like shakiness, sweating, dizziness, or feeling foggy, especially if you take other diabetes meds. Sprinkling pepper on food is unlikely to matter much, but concentrated pippali supplements are worth mentioning to your pharmacist or doctor so they can keep an eye on your numbers.

From our pharmacist’s overview on the Semaglutide page
“Semaglutide (Ozempic, Rybelsus, Wegovy) lowers blood sugar, and so do most of the 256 supplements on this page, which is why I read this list with real vigilance rather than a shrug. Nearly all of the entries either add to its glucose-lowering effect and raise the risk of hypoglycemia, or push sugar the other way and blunt it, as niaci…”

Indian long pepper (pippali) is a spice long used in Ayurvedic medicine and is best known for its piperine content, which may increase how well the body absorbs certain other substances. About Semaglutide
What Is Semaglutide Used For?
Brand names: Ozempic, Rybelsus, Wegovy
Semaglutide is available as Wegovy (injection and tablet) and Ozempic (injection), and each product has its own approved uses. Wegovy injection is approved — alongside diet and exercise — to reduce excess body weight in adults and in children aged 12 and older with obesity, to lower the risk of serious heart events (heart attack, stroke, or cardiovascular death) in adults with obesity or overweight who also have established heart disease, and for treatment of a liver condition called MASH (metabolic dysfunction-associated steatohepatitis, formerly called NASH) with moderate-to-advanced scarring in adults. Wegovy tablets share the cardiovascular risk reduction and weight management uses in adults, but are not approved for MASH or for use in children. Ozempic injection is approved to improve blood sugar control in adults with type 2 diabetes (alongside diet and exercise) and to reduce the risk of major cardiovascular events in adults with type 2 diabetes who also have established heart disease.
All forms are used alongside a reduced-calorie diet and increased physical activity. Ozempic is not approved for type 1 diabetes. The MASH approval for Wegovy injection is an accelerated approval, meaning continued approval may depend on results from a confirmatory study.

Semaglutide Is Linked to Lower Levels of One Nutrient
Natural Medicines records Semaglutide as a medicine that can lower these. This is not an interaction with Indian Long Pepper — it is a separate effect of the medicine, and it is a reason some people are advised to supplement rather than to avoid one:
