LEVODOPA
Advise patients to separate doses of levodopa and iron as much as possible. There is some evidence in healthy people that iron forms chelates with levodopa, reducing the amount of levodopa absorbed by around 50%. The clinical significance of this hasn't been determined.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here is the practical issue: when iron and your levodopa/carbidopa land in your gut at the same time, the iron can bind to the levodopa and grab onto it. That chelation means less of the medicine actually gets absorbed into your bloodstream. In healthy volunteers, this dropped levodopa absorption by roughly half, though we honestly don't know yet how much that changes things for any one person.
For Parkinson's, less levodopa getting in can mean your symptoms aren't as well controlled, more stiffness, tremor, or slowness, or feeling like a dose just isn't kicking in. The simple fix is timing. Try to space your iron and your levodopa/carbidopa as far apart as you can, and check the best schedule with your pharmacist or doctor before changing anything.
