Here is the interaction we found between these two, with the evidence behind it.
METHYLDOPA (Aldomet)
Advise patients to separate methyldopa and iron doses by at least 2 hours. Iron can decrease the absorption of methyldopa from the gastrointestinal tract through chelation, resulting in increases in blood pressure.
Interaction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports..
Here's the practical part for your combo pill. One of its ingredients, methyldopa, is your blood pressure medicine, and iron can grab onto it in your stomach (a process called chelation). When that happens, less methyldopa gets absorbed, so you may be getting a weaker dose than intended. The likely result is that your blood pressure creeps back up, since the medicine isn't fully doing its job.
The fix is simple timing. Take your iron and this medication at least 2 hours apart so they don't bind together in your gut. The hydrochlorothiazide (water pill) part isn't the concern here. Please check with your pharmacist or doctor before changing anything, especially if you monitor your blood pressure at home.
