The ketogenic diet involves a very low intake of carbohydrates, which can result in decreased blood glucose levels in patients taking antidiabetes drugs. In some patients, the need for antidiabetes drugs is reduced. Hypoglycemia has also occurred in some individuals without diabetes, although this is uncommon.

Lyumjev (insulin lispro-aabc) is a fast-acting mealtime insulin that lowers your blood sugar by helping your cells take in glucose. Here's the thing to watch: a ketogenic diet cuts carbs way down, so there is less sugar coming in for that insulin to cover. Stack a strong blood-sugar-lowering insulin on top of very low carb meals and your levels can drop too far.

That means a real chance of hypoglycemia, with symptoms like shakiness, sweating, a racing heart, confusion, or feeling lightheaded. The evidence here is modest and the effect is possible rather than certain, but it matters because mealtime insulin doses are usually matched to the carbs you eat. Please don't change your dose on your own. Talk with your doctor or pharmacist first, since your insulin needs may need adjusting and closer glucose checks.
