Diltiazem falls under ANTIHYPERTENSIVE DRUGS — the group Kiwi interacts with.

Here's the simple version: diltiazem already does the job of lowering your blood pressure and easing the heart's workload. Eating kiwi has been shown in some studies to gently nudge blood pressure down too. Put the two together and, in theory, your numbers could dip a little lower than expected. There's no clash with how your body breaks diltiazem down, so it's not about the drug building up. It's just two mild blood pressure helpers stacking.
If your pressure drops too far, you might feel dizzy, lightheaded, tired, or wobbly when you stand up. To be clear, this is a theoretical concern, and normal food amounts of kiwi are very unlikely to cause trouble. Still, if you eat a lot of it or notice these symptoms, check in with your pharmacist or doctor.

Oral extended-release diltiazem (capsules and tablets) treats high blood pressure (hypertension). It is also used for chronic stable angina, which is chest pain from the heart working harder than its blood supply allows. It can be used alone or with other blood pressure medicines.
Some extended-release capsule products, such as Cartia, Cartia Xt and certain generic diltiazem capsules, are also approved for angina due to coronary artery spasm. The intravenous (IV) injection is a different use: it gives temporary control of a rapid heart rate in atrial fibrillation or atrial flutter, and rapidly converts paroxysmal supraventricular tachycardia (PSVT) to normal rhythm.
