Here's the simple version: iloprost (Ventavis) works by relaxing and widening your blood vessels, including the ones in your lungs, to ease the pressure of pulmonary arterial hypertension. Real licorice (the kind with glycyrrhizin) tends to do the opposite over time. It makes your body hold onto sodium and water and lose potassium, which can nudge your blood pressure up. So the two work against each other, and licorice could blunt some of the benefit you're getting from iloprost.

This is a moderate, possible concern, and it mostly applies to regular licorice taken in larger amounts or for a while, not a quick herbal tea or a little candy. Deglycyrrhizinated (DGL) products don't carry this risk. Since PAH is serious, please check with your pharmacist or doctor before using any licorice supplement.

Licorice root is a traditional remedy used for sore throats, coughs, and digestive complaints, but solid human evidence is limited for most uses. Regular licorice contains glycyrrhizin, which can raise blood pressure and lower potassium when taken in large amounts or for a long time, so deglycyrrhizinated (DGL) products are often preferred. Aurlumyn (iloprost injection, given intravenously) is approved to treat severe frostbite in adults, specifically to reduce the risk of losing fingers or toes (digit amputations). Its effectiveness was established in young, healthy adults who experienced frostbite at high altitudes.
