CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Sofosbuvir, Velpatasvir falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Licorice interacts with.
Pharmacokinetic research shows that the licorice constituent glycyrrhizin, taken in a dosage of 150 mg orally twice daily for 14 days, modestly decreases the area under the concentration-time curve of midazolam by about 20%. Midazolam is a substrate of CYP3A4, suggesting that glycyrrhizin modestly induces CYP3A4 activity. Animal research also shows that licorice extract from the species G. uralensis induces CYP3A4 activity. However, licorice extract from G. glabra species appear to inhibit CYP3A4-induced metabolism of testosterone in vitro. It is thought that the G. glabra inhibits CYP3A4 due to its constituent glabridin, which is a moderate CYP3A4 inhibitor in vitro and not present in other licorice species. Until more is known, licorice should be used cautiously in people taking CYP3A4 substrates.

Here's the gist with Epclusa (sofosbuvir/velpatasvir) and licorice. The velpatasvir part is handled, in part, by an enzyme called CYP3A4, and licorice can nudge that enzyme. The tricky thing is that licorice can go either way depending on the species and the product. Some forms appear to speed the enzyme up (which could lower drug levels a bit), while others may slow it down (raising levels). Either way, this is mostly theoretical and based on limited research, so I wouldn't lose sleep over it.
Still, because you're treating hepatitis C and you want full virus clearance, it makes sense not to gamble with anything that could lower your levels. Licorice also carries its own baggage, raising blood pressure and dropping potassium. I'd run it by your pharmacist or doctor before adding it.

P-GLYCOPROTEIN SUBSTRATES

Here's the gist for your hepatitis C combo, Epclusa (sofosbuvir and velpatasvir). Both ingredients ride through your body on a transporter called P-glycoprotein, which acts like a doorway controlling how much gets absorbed from your gut. Lab studies suggest licorice may speed up that doorway, which could mean less of the medicine actually gets into your bloodstream.
Why that matters: with hepatitis C treatment, you really want full drug levels so the virus is cleared completely. Lower levels could, in theory, make treatment work less well. I want to be honest though, this concern comes only from test-tube research, not from people, so it's quite uncertain. To be safe, check with your pharmacist or specialist before adding licorice supplements while you're on Epclusa.

In vitro, licorice extract from the species G. glabra and G. uralensis inhibits CYP2C8 isoenzymes. Theoretically, these species of licorice might increase levels of drugs metabolized by CYP2C8; however, this interaction has not yet been reported in humans.

Here's the gist for your Epclusa (sofosbuvir/velpatasvir): in lab dishes, licorice extract seems to slow down a liver enzyme called CYP2C8. In theory, slowing that enzyme could let some drugs build up a bit higher than usual. With Epclusa, that might mean slightly more of the medicine sticking around, which could nudge up common side effects like headache, tiredness, or nausea.
The important thing to know is that this is theoretical right now. It comes only from test-tube studies, and it has never actually been reported in people. So I wouldn't lose sleep over it. Still, since finishing your hepatitis C course as prescribed really matters, it's worth a quick chat with your pharmacist or doctor before adding any licorice supplement.

In vitro research shows that licorice extract and glabridin, a licorice constituent, inhibit CYP2B6 isoenzymes. Licorice extract from the species G. uralensis seems to inhibit CYP2B6 isoenzymes to a greater degree than G. glabra extract in vitro. Theoretically, these species of licorice might increase levels of drugs metabolized by CYP2B6; however, these interactions have not yet been reported in humans.

Here's the honest picture for Epclusa (sofosbuvir/velpatasvir): the concern on file is that licorice may slow down a liver enzyme called CYP2B6, which in theory could let certain drugs build up. The catch is that this is only laboratory and test-tube data, not something seen in real patients, and CYP2B6 isn't a major pathway for clearing this particular combo. So I wouldn't lose sleep over it.
My bigger, more practical worry with licorice is its own glycyrrhizin, which can raise blood pressure and lower potassium when used regularly. That matters more here than the enzyme angle. If you take Epclusa, check with your pharmacist or doctor before using licorice supplements, and DGL products avoid that glycyrrhizin issue.

Regular licorice contains glycyrrhizin, which can raise blood pressure and lower potassium when taken in large amounts or for a long time, so deglycyrrhizinated (DGL) products are often preferred. What Is Sofosbuvir and Velpatasvir Used For?
Brand name: Epclusa
Sofosbuvir and velpatasvir treats chronic hepatitis C virus (HCV) infection caused by genotypes 1, 2, 3, 4, 5, or 6 — covering all the major strains. It is approved for adults and children 3 years of age and older, whether or not they have cirrhosis (liver scarring).
It is used on its own in patients without cirrhosis or with compensated cirrhosis (where the liver is scarred but still functioning). In patients with decompensated cirrhosis (more advanced liver disease where the liver is no longer coping), it is used in combination with another medicine called ribavirin. Epclusa is a brand that is also available as oral pellets for younger children.
