CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Why this applies to Vinblastine

Vinblastine falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Licorice interacts with.

Theoretically, licorice might increase or decrease levels of drugs metabolized by CYP3A4.
Pharmacokinetic research shows that the licorice constituent glycyrrhizin, taken in a dosage of 150 mg orally twice daily for 14 days, modestly decreases the area under the concentration-time curve of midazolam by about 20%. Midazolam is a substrate of CYP3A4, suggesting that glycyrrhizin modestly induces CYP3A4 activity. Animal research also shows that licorice extract from the species G. uralensis induces CYP3A4 activity. However, licorice extract from G. glabra species appear to inhibit CYP3A4-induced metabolism of testosterone in vitro. It is thought that the G. glabra inhibits CYP3A4 due to its constituent glabridin, which is a moderate CYP3A4 inhibitor in vitro and not present in other licorice species. Until more is known, licorice should be used cautiously in people taking CYP3A4 substrates.

Here's the situation with licorice and your vinblastine. Vinblastine is handled by a liver enzyme called CYP3A4, and licorice can nudge that enzyme in either direction depending on the type. Some licorice (the glycyrrhizin part) seems to speed the enzyme up a little, which could clear vinblastine faster and slightly weaken it. Other licorice (the glabridin in G. glabra) may slow the enzyme in lab tests, which could let vinblastine build up and raise the risk of its usual side effects like low blood counts, infections, nerve tingling, and constipation.

Honestly, this is mostly theoretical and the human evidence is thin, but vinblastine is potent chemo, so I wouldn't mix in licorice on your own. Please run it by your oncologist or pharmacist first.

Here's the idea with licorice and your vinblastine. Vinblastine is moved around your body by a kind of cellular pump called P-glycoprotein. Lab studies (in test tubes and animals, not people) suggest licorice may rev up that pump, which could push more vinblastine back out and lower how much actually gets absorbed and stays in your system. In theory, that might make this chemotherapy work a little less well.

I want to be honest: this is very early, theoretical evidence, so I'm not ringing alarm bells. Still, vinblastine is serious chemo with a tight margin, so I wouldn't add a licorice supplement without checking first. Please run it by your oncologist or pharmacist before using any licorice product, including DGL.

Licorice root is a traditional remedy used for sore throats, coughs, and digestive complaints, but solid human evidence is limited for most uses. Regular licorice contains glycyrrhizin, which can raise blood pressure and lower potassium when taken in large amounts or for a long time, so deglycyrrhizinated (DGL) products are often preferred. Talk to your pharmacist or doctor before using it, especially if you have high blood pressure, heart, or kidney problems.

What Is Vinblastine Used For?

Vinblastine Sulfate Injection is used in the treatment of a range of cancers. Conditions that tend to respond well include generalized Hodgkin's disease, lymphocytic lymphoma, histiocytic lymphoma, advanced mycosis fungoides (a type of skin lymphoma), advanced testicular cancer, Kaposi's sarcoma, and Letterer-Siwe disease (a rare immune condition also called histiocytosis X).

It may also be used for cancers that respond less reliably, such as choriocarcinoma resistant to other chemotherapy drugs, and breast cancer that has not responded to hormonal therapy. Vinblastine is often given as part of a combination chemotherapy regimen alongside other cancer medicines to improve response rates.
