Here is the interaction we found between these two, with the evidence behind it.
DIURETIC DRUGS
Why this applies to Hydrochlorothiazide, Lisinopril
Hydrochlorothiazide, Lisinopril falls under DIURETIC DRUGS — the group Lovage interacts with.
Some diuretics include chlorothiazide (Diuril), chlorthalidone (Thalitone), furosemide (Lasix), hydrochlorothiazide (HCTZ, Hydrodiuril, Microzide), and others.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: your blood pressure pill is a two-in-one. The lisinopril relaxes your blood vessels, and the hydrochlorothiazide is a water pill that helps your body shed extra salt and fluid through your urine. Lovage is traditionally used as a mild herb that also nudges the kidneys to release more water, what we call an aquaretic effect.
So in theory, stacking lovage on top of your water pill could add up and pull out a little more fluid than expected. That might leave you feeling lightheaded, extra thirsty, or low on energy, especially when standing up quickly. I want to be honest though: this concern is based on how the herb behaves in the lab, not on real patient studies, so it's a maybe, not a sure thing. Cooking amounts are unlikely to matter much. Before adding a concentrated lovage supplement, just run it by your pharmacist or doctor.

From our pharmacist’s overview on the Hydrochlorothiazide, Lisinopril page
“I would not add a supplement casually alongside hydrochlorothiazide and lisinopril (Zestoretic, Prinzide). This combination lowers blood pressure with a water pill plus an ACE inhibitor, and most interacting ingredients either drag pressure down too far or push back against the drug, while smaller groups strain the kidneys, drain potas…”

About Lovage
What is Lovage?
- Digestive upset and bloating
- Mild urinary tract flushing
- Water retention (mild diuretic)
- Coughs and congestion
- Menstrual discomfort
- Culinary flavoring
Lovage is a celery-like herb traditionally used for digestion, bloating, and as a mild diuretic to flush the urinary tract. Human evidence for these uses is very limited, and while culinary amounts are generally well tolerated, concentrated supplements and the essential oil should be used cautiously. Pregnant and breastfeeding people should avoid it.
About Lisinopril and Hydrochlorothiazide
What Is Lisinopril and Hydrochlorothiazide Used For?
Brand names: Zestoretic, Prinzide
Lisinopril and hydrochlorothiazide tablets — sold under the brand name Zestoretic and as the generic combination — are used to treat high blood pressure (hypertension). Lowering blood pressure reduces the risk of serious cardiovascular events, including strokes and heart attacks.
This combination is not intended as a first-line starting treatment. It is typically used when a single blood pressure medication alone has not been enough to reach target blood pressure goals.
