Here's the gist: Madagascar periwinkle may have its own blood-sugar-lowering effect. So far that's only been seen in diabetic rats, not in people, so think of this as a theoretical concern rather than something we know happens. Still, if the herb does nudge your glucose down while you're already taking ertugliflozin (Steglatro), the two together could push your blood sugar lower than intended.

Ertugliflozin alone rarely causes low blood sugar, but stacking another glucose-lowering agent on top can raise that risk, especially if you also take insulin or a sulfonylurea. Watch for shakiness, sweating, dizziness, or confusion. Honestly, I'd skip the raw plant entirely since it's toxic, but please loop in your pharmacist or doctor before combining anything new with your diabetes meds.
