Here's the gist: in some animal studies, Madagascar periwinkle on its own seemed to push blood sugar down. Metformin already works to lower your blood sugar, so the worry is that stacking the two could lower it a little too much. With metformin alone, true low blood sugar (hypoglycemia) is uncommon, but adding something else that nudges sugar down could tip the balance, especially if you also take insulin or other diabetes meds.

I want to be honest, though: this concern comes only from lab and rat research, not human studies, so it's theoretical at this point. Watch for shakiness, sweating, dizziness, or confusion, and keep an eye on your glucose readings. Honestly, the bigger issue is that the raw plant is toxic and not safe to take. Please check with your doctor or pharmacist before trying it.

Metformin is used to improve blood sugar (glucose) control in people with type 2 diabetes mellitus. It is always used together with diet and exercise, not instead of them.

Metformin hydrochloride tablets and oral solution are approved for adults and children 10 years and older. Metformin hydrochloride extended-release tablets — including brands such as Fortamet, Glumetza, and Metformin Hydrochloride Extended Release — are approved for adults only.
