Here is the interaction we found between these two, with the evidence behind it.
CNS DEPRESSANTS
Why this applies to Perampanel
Perampanel falls under CNS DEPRESSANTS — the group Motherwort interacts with.
Motherwort has been reported to cause CNS depression and sedation.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the simple version: Perampanel already tends to make people drowsy, and it carries a known warning for sleepiness, dizziness, and sometimes trouble with balance or coordination. Motherwort has been reported to have a calming, sedating effect of its own. So when you put the two together, the worry is that those drowsy feelings could stack up and leave you more groggy than usual.
I'll be honest with you, this concern is theoretical, based on how each one behaves rather than on solid studies in people. It isn't about your liver breaking the drug down differently, just an additive calming effect. Still, since Perampanel can affect alertness and mood, it's worth running this by your pharmacist or doctor before adding motherwort, especially before driving.

From our pharmacist’s overview on the Perampanel page
“Perampanel (Fycompa) is a seizure medicine that already causes drowsiness and dizziness on its own, which is why I treat this list of 230 interacting supplements with real care. Most entries do one of two things: they add to its sedating effect, as alcohol and kava do, or they change how the liver handles it.”
