Here's the basic idea with neem and nelarabine. Nelarabine is a chemotherapy drug used for certain leukemias and lymphomas, and one of the ways it works ties in with the immune system. In animal studies, neem appears to stimulate the immune system. Because of that, the concern on file is that neem could, in theory, work against the way some immune-affecting cancer treatments are meant to act.
I want to be honest about how soft this evidence is. This is based only on lab and animal research, not human studies, so it's very much a theoretical caution rather than a proven problem. Still, when you're going through cancer treatment, it's smart to keep things simple. Please run any neem product (oral, oil, or extract) past your oncologist or pharmacist before using it.

What Is Nelarabine Injection Used For?
Brand name: Atriance
Nelarabine injection (brand name Arranon) is used to treat two aggressive blood cancers: T-cell acute lymphoblastic leukemia (T-ALL) and T-cell lymphoblastic lymphoma (T-LBL). It is approved for adult and pediatric patients age 1 year and older whose disease has not responded to — or has come back after — at least two prior chemotherapy regimens.
Nelarabine injection may also be given in combination with multi-agent chemotherapy as part of treatment for newly diagnosed T-ALL and T-LBL in certain settings, as studied in clinical trials.
