Resmetirom falls under CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES — the group Neem interacts with.

Theoretically, neem leaf extract might increase the levels and clinical effects of CYP2C8 substrates.
In vitro research shows that neem leaf methanol extract inhibits CYP2C8 enzymes. So far, this reaction has not been reported in humans.

Here's the short version for anyone taking Rezdiffra (resmetirom) for their liver. Your body clears this medicine largely through a liver enzyme called CYP2C8. In test-tube studies, neem leaf extract appears to slow that same enzyme down. If it does the same thing inside a person, resmetirom could clear more slowly and build up a bit, which might mean stronger effects and more of its usual side effects, like nausea, diarrhea, or itching.

I want to be honest about how weak this is: it's only lab data, and nothing like it has actually been reported in people. So please don't panic. Still, since resmetirom is newer and neem seed products can be harsh anyway, it's worth a quick chat with your pharmacist or doctor before combining them.
