This combination pill already does a lot to slow down your nervous system. The butalbital (a barbiturate) and the codeine (an opioid) both cause drowsiness, and the worry here is simply piling more sedation on top. Phellodendron contains berberine, and in animal studies berberine seemed to have a calming, sedative effect. So in theory, taking the two together could leave you feeling extra sleepy, foggy, dizzy, or slow to react.

I want to be honest that this is a theoretical concern based on lab and animal data, not strong human studies, so it may not amount to much. Still, since both can affect alertness and breathing, please check with your pharmacist or doctor before pairing them, and don't drive until you know how you feel.

Theoretically, phellodendron might increase serum levels of drugs metabolized by CYP2D6.
Phellodendron contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, phellodendron might also inhibit CYP2D6.

Here's something worth knowing about this combo pill. The codeine part is what we call a prodrug, meaning it doesn't do much until your body converts it into morphine, and the enzyme that does that conversion is CYP2D6. Phellodendron (through its berberine content) may slow down CYP2D6.

So in this case, blocking that enzyme could mean less of your codeine gets turned into active morphine, which may leave you with weaker pain relief than expected. The acetaminophen and butalbital pieces aren't really affected here. Keep in mind this is only a theoretical concern based on lab and early research, not strong human proof. Still, if you take this medicine for pain, it's worth a quick chat with your pharmacist or doctor before adding phellodendron.
