Trazodone HCl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Phellodendron interacts with.
Phellodendron contains berberine. In vitro research and preliminary clinical evidence show that berberine can inhibit CYP2D6. Theoretically, phellodendron might also inhibit CYP2D6.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: your body uses an enzyme called CYP2D6 to help clear trazodone. Phellodendron contains berberine, which lab studies and some early human data suggest can slow that enzyme down. If trazodone is cleared more slowly, a bit more of it can linger in your system, which may nudge its usual effects up a notch. Think more drowsiness or grogginess, dizziness or lightheadedness when you stand, dry mouth, or feeling unusually sedated.
I want to be honest about the strength here: this is theoretical, based mainly on lab work, not on solid real-world reports with trazodone. So don't panic. Just don't start phellodendron alongside your trazodone without first checking with your pharmacist or doctor, especially concentrated supplement doses.

Phellodendron contains berberine. In vitro research and preliminary clinical research show that berberine moderately inhibits CYP3A4. Theoretically, phellodendron might also inhibit CYP3A4.

Here's the gist: your body relies on a liver enzyme called CYP3A4 to clear trazodone out of your system. Phellodendron contains berberine, which lab and early human studies suggest can partly block that same enzyme. If it slows that breakdown, more trazodone could linger in your blood, which may mean stronger effects than usual.
With trazodone, that could show up as extra drowsiness, dizziness, low blood pressure when you stand, or a groggy, hungover feeling the next morning. To be fair, this concern is theoretical, based mostly on how berberine behaves in the lab, not on solid real-world reports. Still, since both can make you sleepy, it's worth a quick chat with your pharmacist or doctor before combining them.
