Metformin, Sitagliptin falls under ANTIDIABETES DRUGS — the group Phellodendron interacts with.

Theoretically, phellodendron may increase the risk of hypoglycemia when taken with antidiabetes drugs.
Phellodendron contains berberine. Clinical research shows that berberine may lower blood glucose levels. Theoretically, phellodendron might also lower blood glucose levels.

Here's the thing to keep in mind with Janumet (the metformin and sitagliptin combo): this medicine is already working hard to bring your blood sugar down. Phellodendron contains berberine, and in studies berberine can lower blood sugar on its own. So when you stack the two together, the worry is they could add up and push your numbers down too far.

I'll be honest, this is a theoretical concern based on how berberine behaves, not strong proof of trouble in real life. Still, low blood sugar is no fun. Watch for shakiness, sweating, dizziness, headache, or feeling suddenly hungry or confused. If you'd like to try Phellodendron, please loop in your pharmacist or doctor first so they can keep an eye on your readings and adjust if needed.

Theoretically, phellodendron might increase the therapeutic and adverse effects of metformin.
Phellodendron contains berberine. In vitro and animal studies show that berberine can increase the systemic exposure and half-life of metformin, potentially increasing metformin's effects and side effects. This interaction seems to be most apparent when berberine is administered 2 hours prior to metformin. Taking berberine and metformin at the same time does not appear to increase systemic exposure to metformin. It is unclear if phellodendron might have this same effect.

Janumet combines two diabetes medicines: metformin and sitagliptin. The interaction we have on file is really about the metformin half. Phellodendron contains berberine, and lab and animal studies suggest berberine may slow how your body clears metformin, letting a bit more of it hang around. In theory, that could nudge metformin's effects (and side effects) a little higher.

For you, that might mean more of metformin's usual issues, like upset stomach, nausea, or loose stools, and possibly lower blood sugar than expected. Keep in mind this is only theoretical, based on pharmacology, not proven in people. Interestingly, the effect seemed strongest when berberine was taken a couple hours before metformin, not together. Please check with your pharmacist or doctor before combining them.
