Here's the gist with Janumet (the combo of metformin and sitagliptin): this medicine already works to lower your blood sugar. In animal studies, rehmannia root seemed to lower blood sugar on its own too. So the worry is simple addition. If both nudge your glucose down at the same time, you could end up lower than intended, especially if you skip meals, exercise hard, or drink alcohol.

To be fair, this is a theoretical concern based mostly on lab and animal work, not solid human data, so I wouldn't lose sleep over it. Still, it's worth watching for shakiness, sweating, dizziness, or confusion, which can signal low blood sugar. Don't change anything on your own. Just give your pharmacist or doctor a heads-up before adding rehmannia.
