Metformin XR falls under ANTIDIABETES DRUGS — the group Picrorhiza interacts with.

Evidence from animal research suggests that an extract of picrorhiza can reduce fasting and non-fasting blood sugar levels. Theoretically, picrorhiza might have additive effects with antidiabetes drugs and increase the risk of hypoglycemia. Monitor blood glucose levels closely. Dose adjustments might be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (DiaBeta, Glynase PresTab, Micronase), insulin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the gist for Metformin XR: this isn't about your body breaking the drug down differently. Instead, picrorhiza on its own seems to nudge blood sugar downward, at least in animal studies. So when you stack it with metformin, which also lowers blood sugar, the two could add up and push your numbers a little too low.

With metformin alone, true lows are uncommon, but adding another sugar-lowering agent shifts the math. Watch for shakiness, sweating, dizziness, or feeling unusually hungry or foggy. I'll be honest though, this is a theoretical concern based mostly on lab and animal work, not strong human data. If you'd like to try picrorhiza, keep an eye on your home glucose readings and check with your pharmacist or doctor first so they can guide any adjustments.
