Troglitazone falls under ANTIDIABETES DRUGS — the group Picrorhiza interacts with.

Evidence from animal research suggests that an extract of picrorhiza can reduce fasting and non-fasting blood sugar levels. Theoretically, picrorhiza might have additive effects with antidiabetes drugs and increase the risk of hypoglycemia. Monitor blood glucose levels closely. Dose adjustments might be necessary. Some antidiabetes drugs include glimepiride (Amaryl), glyburide (DiaBeta, Glynase PresTab, Micronase), insulin, pioglitazone (Actos), rosiglitazone (Avandia), and others.

Here's the gist: troglitazone (Rezulin) is a diabetes medicine that helps your body use insulin better and lowers blood sugar. Picrorhiza, in animal studies, also seems to nudge blood sugar down a bit. Put the two together and the worry is simply additive, meaning your sugar could drop lower than you intend.

If that happens, you might notice signs of low blood sugar such as shakiness, sweating, dizziness, a fast heartbeat, or feeling confused or unusually hungry. To be fair, this is a theoretical concern based mostly on lab and animal data, so it's far from certain. Still, if you ever combine these, check your blood sugar more closely and talk with your pharmacist or doctor before changing anything.
