Dronabinol falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Polypodium Leucotomos interacts with.

Theoretically, Polypodium leucotomos might increase levels of drugs metabolized by CYP3A4.
In vitro and animal research suggests that Polypodium leucotomos can decrease the metabolism of midazolam by a mechanism possibly related to the inhibition of CYP3A4. However, a small study in humans suggests that taking Polypodium leucotomos with midazolam does not affect the metabolism of midazolam.

Here's the gist for dronabinol (Marinol, Syndros): your liver uses an enzyme called CYP3A4 to help clear it out. The idea on paper is that Polypodium leucotomos might slow that enzyme down a bit, which could let dronabinol build up slightly. If that happened, you might notice more of its usual effects, things like drowsiness, dizziness, a fast heartbeat, or feeling lightheaded or "high."

That said, I don't want to overstate this. The concern comes mostly from lab and animal work, and when researchers actually tested a similar CYP3A4 drug (midazolam) in people taking this fern extract, they saw no real change. So this is rated minor and unlikely. Still, since dronabinol already causes plenty of drowsiness on its own, it's worth a quick mention to your pharmacist or doctor before combining them.

What Is Dronabinol Used For?

Brand names: Marinol, Syndros

Dronabinol — available as capsules (Marinol, Dronabinol) and as an oral solution (Syndros, Dronabinol oral solution) — is approved for two conditions in adults. First, it treats anorexia (severely reduced appetite) associated with weight loss in patients with AIDS. Second, it controls nausea and vomiting caused by cancer chemotherapy in patients who have not gotten enough relief from standard anti-nausea treatments.

Both uses apply to all available formulations, but dosing timing and meal instructions differ between the capsule and oral solution products. Always follow the specific instructions for the product you have been prescribed.
