Fenfluramine falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Pomegranate interacts with.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP2D6.
In vitro, pomegranate juice inhibits CYP2D6. However, the clinical significance of this potential interaction in humans is not known.

Here's the gist for fenfluramine (Pondimin). Your body partly clears this medicine using a liver enzyme called CYP2D6. In lab-dish testing, pomegranate has slowed that enzyme down. If that happened in real life, fenfluramine could build up a bit higher than usual, which in theory might mean stronger effects or more side effects like sleepiness, low appetite, tiredness, or a faster heartbeat.

The honest part: this is based only on test-tube work, not on people, so we really don't know if it matters in everyday life. Fenfluramine is not a prodrug, so it would lean toward more drug, not less. Eating pomegranate or having some juice now and then is unlikely to be a problem, but check with your pharmacist or doctor before using concentrated pomegranate extracts.

What Is Fenfluramine Used For?

Brand name: Pondimin

Fintepla (fenfluramine oral solution) is approved to treat seizures associated with Dravet syndrome (DS) and Lennox-Gastaut syndrome (LGS) in patients 2 years of age and older. It is used alongside other antiseizure medications.

Both conditions are severe, often hard-to-control forms of epilepsy that typically begin in childhood. Fintepla is available only through a restricted program called the Fintepla REMS because of serious heart-related risks.
