Fomoterol falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Pomegranate interacts with.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP2D6.
In vitro, pomegranate juice inhibits CYP2D6. However, the clinical significance of this potential interaction in humans is not known.

Here's the short version on pairing pomegranate with your formoterol (Foradil) inhaler. Your body uses a liver enzyme called CYP2D6 to help clear formoterol. In a test tube, pomegranate juice slowed that enzyme down. If the same thing happened in people, a little more formoterol could linger in your system, which might mean more of its usual effects like a fast or pounding heartbeat, jitteriness, shakiness, or a headache.

The honest part: this comes only from lab studies, not real patients, so we truly don't know if it matters in everyday life. Enjoy pomegranate as a food without much worry, but check with your pharmacist or doctor before using concentrated pomegranate extracts.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP2C9.
Some animal and in vitro research shows that pomegranate juice inhibits intestinal, but not hepatic, CYP2C9 isoenzyme activity. However, clinical research shows that neither pomegranate juice nor pomegranate extract have a significant effect on CYP2C9 activity in humans.

Here's the short version: this is a very low-concern pairing. The idea on file is that pomegranate might slow down an enzyme called CYP2C9, which could in theory let certain drugs build up. With formoterol (Foradil), that would mean a bit more of the medicine hanging around, which could nudge up its usual side effects like a fast or pounding heartbeat, shakiness, or feeling jittery.

But the real-world evidence doesn't support this being a problem. Human studies show pomegranate juice and extract don't meaningfully change CYP2C9 activity, and any effect seen in lab and animal work was in the gut, not the liver. Enjoying pomegranate as a food or juice should be fine. If you use concentrated extracts or notice new symptoms, just check with your pharmacist or doctor.

What Is Formoterol Oral Inhalation Used For?

Brand name: Foradil

Formoterol fumarate inhalation solution is used for the long-term maintenance treatment of airway narrowing (bronchoconstriction) in adults with COPD, including chronic bronchitis and emphysema. It is taken twice daily — morning and evening — as a regular, ongoing treatment.

Important limits: This medication is not for relieving sudden COPD flare-ups or breathlessness in the moment, and it is not approved to treat asthma. A separate short-acting inhaler should always be on hand for sudden symptoms.
