Opsynvi combines two blood pressure lowering medicines: macitentan (an endothelin receptor blocker) and tadalafil (the same active drug found in Cialis). Both of these already work to relax and widen your blood vessels. Pomegranate juice has been shown to gently lower blood pressure on its own, so pairing the two could add up to a slightly bigger drop than you expect.

What might that feel like? Dizziness, lightheadedness (especially when you stand up quickly), headache, flushing, or feeling wiped out. This is a theoretical concern based on limited evidence, and an occasional glass of juice is usually mild. Regular, concentrated pomegranate extracts are more likely to matter. Please check with your pharmacist or doctor before making it a daily habit, and let them know if you feel faint.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP2C9.
Some animal and in vitro research shows that pomegranate juice inhibits intestinal, but not hepatic, CYP2C9 isoenzyme activity. However, clinical research shows that neither pomegranate juice nor pomegranate extract have a significant effect on CYP2C9 activity in humans.

You're asking about Opsynvi, which combines macitentan and tadalafil to treat pulmonary arterial hypertension. The idea here is that pomegranate could slow down an enzyme (CYP2C9) that helps clear these drugs, which in theory might let a little more build up in your system. If that happened, you could see more of their usual effects, like headache, flushing, low blood pressure, dizziness, or a stuffy nose.

But here's the reassuring part: in actual human studies, pomegranate juice and extract did not meaningfully change CYP2C9 activity. Any blocking effect seen in the gut showed up only in lab and animal research. So enjoying pomegranate as food or juice is very unlikely to cause a problem. If you're thinking about concentrated extract capsules, just run it by your pharmacist first.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP3A4, but most research suggests this interaction is unlikely to be clinically significant.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Opsynvi combines two medicines, macitentan and tadalafil, and both are handled by a liver enzyme called CYP3A4. Pomegranate has some plant compounds that can slow this enzyme in the lab, and in theory that could let these drugs build up a bit and cause more low blood pressure, dizziness, headache, flushing, or stuffy nose. Neither drug is a prodrug, so slowing the enzyme would tend to increase their effect, not weaken it.

The good news: in real human studies, pomegranate juice did not meaningfully change CYP3A4 medicine levels. So enjoying pomegranate as a food or normal glass of juice is unlikely to be a problem. Concentrated extracts are less studied, so run those by your pharmacist or doctor first.
