CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Siponimod falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Pomegranate interacts with.
Some animal and in vitro research shows that pomegranate juice inhibits intestinal, but not hepatic, CYP2C9 isoenzyme activity. However, clinical research shows that neither pomegranate juice nor pomegranate extract have a significant effect on CYP2C9 activity in humans.
Interaction has been demonstrated in animal or in lab research but has been shown not to occur in humans..
Here's the short version: your body relies on an enzyme called CYP2C9 to break down siponimod, and pomegranate has been looked at as something that might slow that enzyme down. If that happened, siponimod levels could creep up, which in theory could mean more of its effects, like a slower heart rate, headache, dizziness, or higher blood pressure.
The good news is that this concern is mostly theoretical. It comes from lab and animal studies, and when researchers actually tested pomegranate juice and extract in people, they found no meaningful effect on CYP2C9. Enjoying pomegranate as a fruit or juice is very unlikely to cause trouble. Because siponimod is a serious MS medicine, it's still smart to mention concentrated pomegranate extracts to your pharmacist or doctor before starting them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES
2 of 2Siponimod also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the gist for Mayzent (siponimod). Your body partly clears this drug using a liver enzyme called CYP3A4. Some of the polyphenols in pomegranate can slow that enzyme in a test tube, and the worry is that this could let siponimod build up a little, which in theory might mean more side effects like a slower heart rate, headache, or higher blood pressure.
The good news: in actual human studies, pomegranate juice didn't meaningfully change levels of similar CYP3A4 drugs, so this interaction is rated minor and unlikely. Enjoying pomegranate as a food or an occasional glass of juice is very unlikely to be a problem. I'd just be a bit more cautious with concentrated extracts. If you want peace of mind, check with your pharmacist or MS doctor.

Mayzent (siponimod) is approved for adults with relapsing forms of multiple sclerosis (MS). This includes three specific types: clinically isolated syndrome (a first episode of MS-like symptoms), relapsing-remitting MS (periods of attacks followed by recovery), and active secondary progressive MS (a phase where the disease gradually worsens but still has active relapses).
It is taken by mouth once daily and is intended as a long-term treatment to reduce relapses and slow disease progression.
