Valbenazine Tosylate falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Pomegranate interacts with.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP2D6.
In vitro, pomegranate juice inhibits CYP2D6. However, the clinical significance of this potential interaction in humans is not known.

Here's the gist with Ingrezza (valbenazine). One of the enzymes your body uses to break it down is called CYP2D6. In lab (test tube) studies, pomegranate appeared to slow that enzyme down a bit. If that held true in people, it could mean a little more valbenazine hanging around in your system, which might nudge up side effects like drowsiness, dizziness, or a slowed heartbeat.

Now, the honest part: this is only seen in lab work, not in actual patients, so we really don't know if it matters in real life. Enjoying pomegranate as fruit or a normal glass of juice is very unlikely to be a problem. I'd just give your pharmacist or doctor a heads-up before using concentrated pomegranate extracts.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP3A4, but most research suggests this interaction is unlikely to be clinically significant.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the gist: your Ingrezza (valbenazine) is cleared from your body partly by a liver enzyme called CYP3A4. Some of the plant compounds in pomegranate can block that enzyme in a test tube, and if that happened in your body, valbenazine could build up a bit and cause stronger effects like drowsiness, dizziness, or a slower-than-normal heartbeat.

The good news is that this worry mostly stays on paper. When researchers actually gave people pomegranate juice and tested similar CYP3A4 medications, levels didn't change in any meaningful way. So enjoying pomegranate as food or juice is very unlikely to be a problem here. Concentrated pomegranate extracts are less studied, so if you're considering those, it's worth a quick check with your pharmacist or doctor.

What Is Valbenazine Used For?

Brand name: Ingrezza

Valbenazine — sold as Ingrezza and INGREZZA Sprinkle — is approved for two conditions in adults. The first is tardive dyskinesia, a movement disorder that causes uncontrolled, repetitive facial or body movements, often brought on by long-term use of certain psychiatric medications. The second is chorea associated with Huntington's disease, which refers to the involuntary, dance-like jerking movements that are a hallmark of that condition.

Both Ingrezza and INGREZZA Sprinkle are approved for both of these uses.
