Voriconazole falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Pomegranate interacts with.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP2C9.
Some animal and in vitro research shows that pomegranate juice inhibits intestinal, but not hepatic, CYP2C9 isoenzyme activity. However, clinical research shows that neither pomegranate juice nor pomegranate extract have a significant effect on CYP2C9 activity in humans.

Here's the short version: your body uses an enzyme called CYP2C9 to help clear voriconazole, and the worry on paper is that pomegranate might slow that enzyme down a bit. If it did, voriconazole could build up slightly, which in theory might mean more of its known side effects like blurry or altered vision, nausea, or changes in liver tests.

But I want to keep this in perspective. The lab and animal hints only touched the gut form of this enzyme, and actual human studies showed pomegranate juice and extract did not meaningfully change CYP2C9 activity. So enjoying pomegranate as food or juice is very unlikely to cause trouble here. If you're considering concentrated pomegranate extract, just run it by your pharmacist or doctor first, since voriconazole levels do matter.

Theoretically, pomegranate might increase levels of drugs metabolized by CYP3A4, but most research suggests this interaction is unlikely to be clinically significant.
Pomegranate contains several polyphenols that have individually been shown to inhibit CYP3A4. However, there is contradictory evidence about the effect of whole pomegranate juice on CYP3A4 activity. In vitro, pomegranate juice significantly inhibits the CYP3A4 enzyme, with comparable inhibition to grapefruit juice. In an animal model, pomegranate juice inhibits CYP3A4 metabolism of carbamazepine and increases levels of carbamazepine by 1.5 times; however, in human volunteers, drinking a single glass of pomegranate juice 240 mL or taking 200 mL daily for 2 weeks does not significantly affect levels of the CYP3A4 substrate midazolam after oral or intravenous administration. Another study in healthy volunteers shows that consuming pomegranate juice 300 mL three times daily for three days also does not significantly affect levels of simvastatin, a CYP3A4 substrate This suggests that pomegranate is unlikely to significantly affect levels of CYP3A4 substrates in humans.

Here's the gist: voriconazole is partly cleared by an enzyme in your liver called CYP3A4. Some of the plant compounds in pomegranate can slow that enzyme down in the lab, and in theory that could let voriconazole build up a bit, which might mean more of its typical side effects like visual changes (blurry vision or seeing brightness), nausea, or liver irritation.

But here's the reassuring part: when researchers actually tested pomegranate juice in people on other CYP3A4 drugs, it didn't meaningfully change their levels. So real-world food and juice amounts are unlikely to cause a problem. Enjoy pomegranate as a food without much worry, and just give your pharmacist a heads-up if you plan to take concentrated pomegranate extracts.

About Voriconazole

What Is Voriconazole Used For?

Brand name: Vfend

Voriconazole treats several serious fungal infections. Across all available forms — tablets, oral suspension, and intravenous (IV) injection — it is used for invasive aspergillosis (a dangerous mold infection), candidemia (Candida fungus in the bloodstream) in patients who are not neutropenic (not severely low on white blood cells), other deep-tissue Candida infections, and serious infections caused by Scedosporium apiospermum or Fusarium species in patients who cannot tolerate or have not responded to other antifungal treatments.

Esophageal candidiasis (a Candida infection of the esophagus, the swallowing tube) is also an approved use for most formulations. Note on the oral tablet form (generic voriconazole tablets): that specific tablet formulation is approved for patients aged 12 to 14 years weighing at least 50 kg, and all patients 15 years and older — not for younger or smaller children. The oral suspension and the IV injection are approved for patients 2 years and older for most of these conditions.
