Theoretically, taking quercetin with antihypertensive drugs might increase the risk of hypotension.

Quercetin can modestly decrease blood pressure in people with mild hypertension. Theoretically, it might have additive blood pressure lowering effects when used with antihypertensive drugs.

Here's the simple version: quercetin can nudge your blood pressure down a little on its own. Since aliskiren (Tekturna) is already working hard to lower your blood pressure by relaxing your blood vessels, stacking the two could add up and drop your numbers more than you want.

If your blood pressure dips too low, you might feel dizzy, lightheaded, tired, or a bit woozy when you stand up quickly. The good news is that this concern is mostly theoretical and based on limited research, and small food amounts of quercetin (apples, onions, berries) are unlikely to cause trouble. Concentrated supplement doses are more worth watching. Before adding a quercetin supplement, check with your pharmacist or doctor, and keep an eye on your home blood pressure readings.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Aliskiren also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Here's the short version: aliskiren is processed in part by a liver enzyme called CYP3A4, and quercetin can tinker with that same enzyme. In most lab studies quercetin tends to slow CYP3A4 down, which could let a bit more aliskiren build up in your system. Aliskiren isn't a prodrug, so more drug around generally means a stronger blood-pressure-lowering effect, possibly with more dizziness, lightheadedness, or feeling washed out.

That said, this is mostly theoretical and based on test-tube and animal work, with mixed results in people, so I wouldn't lose sleep over it. Food amounts of quercetin are very unlikely to matter. Before starting a concentrated quercetin supplement, just run it by your pharmacist or doctor and keep an eye on your blood pressure.

Aliskiren and Tekturna tablets are both approved to treat high blood pressure (hypertension) in adults and in children who are at least 6 years old and weigh 50 kg (about 110 lbs) or more. Lowering high blood pressure reduces the risk of serious events like strokes and heart attacks.
