ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP)

Levofloxacin falls under ORGANIC ANION-TRANSPORTING POLYPEPTIDE SUBSTRATES (OATP) — the group Quercetin interacts with.
In vitro evidence shows that quercetin can inhibit organic anion-transporting peptide (OATP) 1B1-mediated uptake of estrone-3-sulfate and pravastatin. Furthermore, clinical research in healthy males shows that intake of quercetin along with pravastatin increases the AUC of pravastatin by 24%, prolongs its half-life by 14%, and decreases its apparent clearance by 18%, suggesting that quercetin modestly inhibits the uptake of pravastatin in hepatic cells.

Here's the gist for levofloxacin (Levaquin): quercetin can block a transporter called OATP, which helps move certain medicines into cells. If that transporter slows down, a drug it carries can build up a bit more in the body. In theory, that could mean slightly stronger levofloxacin effects, and possibly more of its usual side effects like nausea, diarrhea, dizziness, or trouble sleeping.
I want to be honest about how shaky this is. The real human data we have is for pravastatin, not levofloxacin, and even there the change was modest (about a 24% bump). For levofloxacin this is mostly theoretical. Food amounts of quercetin are very unlikely to matter, but if you take high-dose supplements, check with your pharmacist or doctor first.

QUINOLONE ANTIBIOTICS

In vitro, quercetin binds to the DNA gyrase site on bacteria, which may interfere with the activity of quinolone antibiotics.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the idea behind this pairing. Levofloxacin kills bacteria by latching onto a bacterial enzyme called DNA gyrase. In lab studies, quercetin seems to grab onto that very same spot on the bacteria, so the worry is that it could get in the antibiotic's way and make levofloxacin work a little less well. In other words, the concern is weaker antibiotic activity, not stronger side effects.
I want to be honest about how soft this evidence is. It comes mostly from test-tube work and lower-quality research, so we don't really know if it matters in a person fighting a real infection. Still, because an antibiotic only helps if it fully clears the bug, I'd keep things simple: if you're on levofloxacin for an infection, hold off on high-dose quercetin supplements and check with your pharmacist or doctor first. Quercetin from normal foods is very unlikely to be a problem.

Levofloxacin treats bacterial infections in adults, including pneumonia (both community-acquired and hospital-acquired), skin infections, urinary tract infections, kidney infections, chronic bacterial prostatitis, and sinus infections. It is also used after exposure to inhalational anthrax and for plague.
Because levofloxacin carries significant risks, it should only be used for uncomplicated urinary tract infections, acute bacterial worsening of chronic bronchitis, and acute bacterial sinusitis when no other treatment option is available. Always use it only for infections confirmed or strongly suspected to be bacterial.
