CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES

Phenobarbital, Phenytoin falls under CYTOCHROME P450 2C9 (CYP2C9) SUBSTRATES — the group Quercetin interacts with.
A small clinical study in healthy volunteers shows that taking quercetin 500 mg twice daily for 10 days prior to taking diclofenac, a CYP2C9 substrate, increases diclofenac plasma levels by 75% and prolongs the half-life by 32.5%. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar), a substrate of CYP2C9. Furthermore, laboratory research shows that quercetin inhibits CYP2C9.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist: phenytoin (the Dilantin part of this combo) is partly cleared by a liver enzyme called CYP2C9. Quercetin in concentrated supplement form appears to slow that enzyme down. If it's working less, phenytoin can build up in your blood. Phenytoin has a narrow safety window, so even a modest rise can tip you into side effects like dizziness, unsteady walking, slurred speech, double vision, or drowsiness.
To be fair, the direct evidence here is thin. It's mostly lab work plus a small study using a different drug, so this is more theoretical than proven. Everyday food amounts of quercetin are very unlikely to matter. Before starting a high-dose supplement, please run it by your pharmacist or doctor, since your phenytoin level can be checked with a simple blood test.

CYTOCHROME P450 2C8 (CYP2C8) SUBSTRATES

In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.

Here's the gist on combining quercetin with phenobarbital and phenytoin. The theory is that quercetin can block a liver enzyme called CYP2C8, and if that slows how your body clears these seizure medicines, their levels could creep up. Higher phenytoin levels can cause things like dizziness, unsteady walking, slurred speech, double vision, or drowsiness, and phenobarbital can add to that sedation.
That said, I want to be honest about how shaky this is. This concern comes from lab dish studies only, and human evidence with similar CYP2C8 drugs hasn't shown a real problem. Everyday amounts of quercetin in foods are very unlikely to matter. Phenytoin is a finicky drug where small shifts count, so before starting a high-dose quercetin supplement, just run it by your pharmacist or doctor.
