Quercetin can modestly decrease blood pressure in people with mild hypertension. Theoretically, it might have additive blood pressure lowering effects when used with antihypertensive drugs.

Tarka is a combo pill that pairs trandolapril (an ACE inhibitor) with verapamil (a calcium channel blocker), and both of those work to lower your blood pressure. Quercetin, when taken as a concentrated supplement, may also nudge blood pressure down a little. Stack those together and the effects can add up, so your numbers might drop more than you expect.

If that happens, you could feel dizzy or lightheaded, especially when standing up quickly, along with possible fatigue or a headache. This is mostly theoretical and based on limited research, and the small amounts of quercetin in foods like apples and onions aren't likely to cause trouble. Still, before starting a high-dose supplement, run it by your pharmacist or doctor so they can keep an eye on your readings.

P-GLYCOPROTEIN SUBSTRATES

Trandolapril, Verapamil also falls under P-GLYCOPROTEIN SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of P-glycoprotein substrates.

There is preliminary evidence that quercetin inhibits the gastrointestinal P-glycoprotein efflux pump, which might increase the bioavailability and serum levels of drugs transported by the pump. A small study in healthy volunteers reported that pretreatment with quercetin increased bioavailability and plasma levels after a single dose of cyclosporine (Neoral, Sandimmune). Also, two small studies have shown that quercetin might decrease the absorption of talinolol, a substrate transported by the gastrointestinal P-glycoprotein efflux pump. However, in another small study, several days of quercetin treatment did not significantly affect the pharmacokinetics of saquinavir (Invirase). The reason for these discrepancies is not entirely clear. Until more is known, use quercetin cautiously in combination with P-glycoprotein substrates.

Tarka combines two blood pressure medicines, and the verapamil part is moved by a transporter called P-glycoprotein. Quercetin may block that pump a little. In theory, that could let more verapamil get absorbed, nudging its levels up. If that happens, you might notice more of verapamil's usual effects, like lower blood pressure, dizziness, a slower heartbeat, flushing, swollen ankles, or constipation.

Here's the honest part: this is mostly theoretical, based on small, mixed studies, so it may never cause a real problem for you. Food amounts of quercetin are very unlikely to matter. If you're considering a concentrated supplement, check with your pharmacist or doctor first, and don't change your Tarka on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Trandolapril, Verapamil also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Theoretically, concomitant use might alter the effects and adverse effects of CYP3A4 substrates.
A small clinical study in healthy volunteers shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of a single dose of cyclosporine (Neoral, Sandimmune), a substrate of CYP3A4. Animal research also shows that pretreatment with quercetin increases plasma levels and prolongs the half-life of losartan (Cozaar) and quetiapine (Seroquel), substrates of CYP3A4. Other laboratory research also shows that quercetin inhibits CYP3A4. However, one clinical study shows that quercetin can increase the metabolism of midazolam, a substrate of CYP3A4, and decrease serum concentrations of midazolam by about 24% in some healthy individuals, suggesting possible induction of CYP3A4.

Tarka combines verapamil with trandolapril. The verapamil part is the one to watch here, since your liver clears it using an enzyme called CYP3A4. Quercetin can lightly block that enzyme, which in theory could let more verapamil hang around in your blood. If that happens, you might notice more of verapamil's usual effects, like a slower heartbeat, dizziness, lightheadedness, low blood pressure, swollen ankles, or constipation.

Here's the honest part: this is mostly based on lab and animal studies, and the human data is mixed (quercetin has even sped up one CYP3A4 drug in one study). So it's possible, not certain. Food amounts in fruits and veggies are very unlikely to matter. If you're considering high-dose quercetin pills, just run it by your pharmacist or doctor first.

Theoretically, concomitant use might increase the levels and adverse effects of CYP2C8 substrates.

In vitro research shows that quercetin inhibits CYP2C8. Inhibition of paclitaxel (Taxol) metabolism via CYP2C8 has been reported in vitro. However, a small study in humans found no effect of quercetin on rosiglitazone (Avandia), which is also a CYP2C8 substrate.

Here is the reassuring part: this is theoretical and comes from test-tube work, not real-world cases with this medicine. In fact, a human study using another CYP2C8 drug showed no meaningful effect. Quercetin in everyday foods is very unlikely to matter, though concentrated supplements are less studied. If you plan to take high-dose quercetin, just run it by your pharmacist or doctor first.
