Alvimopan falls under P-GLYCOPROTEIN SUBSTRATES — the group Rabdosia Rubescens interacts with.

Rabdosia rubescens might increase levels of p-glycoprotein substrates. In vitro research shows that Rabdosia rubescens extract inhibits p-glycoprotein levels and activity. Drugs that might be affected include some chemotherapeutic agents (doxorubicin, etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the gist of it. Alvimopan (Entereg) is moved around your body partly by a transporter called p-glycoprotein, which acts like a bouncer that pumps the drug back out of cells. In lab studies, Rabdosia rubescens seems to block that pump. If that happened in people, a little more alvimopan could stay in your system than expected.

Practically speaking, alvimopan is given for a short time in the hospital right after bowel surgery, and it's not a prodrug, so higher levels could mean a bit more drug effect. The good news is this is theoretical and based only on test-tube research, not on real patients, so I wouldn't lose sleep over it. Still, since this is a hospital medication, mention any supplements to your surgical team or pharmacist.

What Is Alvimopan Used For?

Alvimopan capsules are used in hospitals to help the digestive tract recover more quickly after surgery that involves removing part of the bowel (partial bowel resection with primary anastomosis — meaning the remaining ends are reconnected). After this type of surgery, the bowel often temporarily stops working, a condition called postoperative ileus.

This medication is only given during a short hospital stay and is not for long-term or home use.
