Rabdosia rubescens might increase levels of p-glycoprotein substrates. In vitro research shows that Rabdosia rubescens extract inhibits p-glycoprotein levels and activity. Drugs that might be affected include some chemotherapeutic agents (doxorubicin, etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the gist for your Myfembree (relugolix, estradiol, and norethindrone). The relugolix piece is moved around the body by a transporter called P-glycoprotein, which acts like a pump that pushes the drug back out of your cells and gut. In test-tube research, Rabdosia rubescens seems to slow that pump down. In theory, that could let a bit more relugolix get absorbed and build up, which might mean stronger effects or more side effects like hot flashes, headache, mood changes, or spotting.

I want to be honest though: this is based only on lab studies, not real people, so we don't truly know if it matters in everyday life. Since Myfembree is a daily medication you depend on, check with your pharmacist or doctor before adding this herb.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Relugolix, Estradiol, Norethindrone Acetate also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Myfembree combines three active medicines (relugolix, estradiol, and norethindrone), and all of them rely on the liver enzyme CYP3A4 to be broken down. Lab and animal studies suggest a compound in Rabdosia rubescens called oridonin may speed up that enzyme. If that happens in people, your body could clear these hormones faster, which may weaken how well they work.

In real terms, that could mean less control of heavy bleeding or pain, or possibly breakthrough spotting. Keep in mind this is only test-tube and animal evidence, with nothing confirmed in humans, so I wouldn't panic. Still, since this medicine matters for your treatment, please check with your pharmacist or doctor before pairing it with this herb.

This combination is used in premenopausal women. Myfembree is approved for two conditions.

It treats heavy menstrual bleeding linked to uterine fibroids (also called leiomyomas). It also treats moderate to severe pain linked to endometriosis. Use is limited to 24 months because of the risk of bone loss that may not be reversible.
