Rabdosia Rubescens and Rimegepant

These two interact in 2 distinct ways. Each one is explained separately below.

Why Are There 2 Interactions?

Rabdosia Rubescens interacts with whole categories of medication. Rimegepant falls into 2 of those categories, so it can interact with Rabdosia Rubescens in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).

Moderate

P-GLYCOPROTEIN SUBSTRATES

1 of 2

Why this applies to Rimegepant

Rimegepant falls under P-GLYCOPROTEIN SUBSTRATES — the group Rabdosia Rubescens interacts with.

Rabdosia rubescens might increase levels of p-glycoprotein substrates. In vitro research shows that Rabdosia rubescens extract inhibits p-glycoprotein levels and activity. Drugs that might be affected include some chemotherapeutic agents (doxorubicin, etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.
Severity Moderate Likelihood Possible Evidence D

Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.

Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the gist for Nurtec ODT (rimegepant). Your body uses a tiny pump called P-glycoprotein to help move rimegepant out of cells and clear it. In test-tube studies, Rabdosia rubescens seems to slow that pump down. If the pump works less, more rimegepant could hang around in your system, which may bump up its usual side effects, things like mild nausea or stomach upset.

The important part: this is only lab-based evidence, not proof from people taking both together, so I wouldn't lose sleep over it. Still, since rimegepant's safety with herbs hasn't been well studied, please check with your pharmacist or doctor before pairing the two, and never adjust your migraine medicine on your own.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

2 of 2

Rimegepant also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Dr. Brian Staiger, PharmD, BCPS
Dr. Brian Staiger explains PharmD, Clinical Pharmacist

Here's the simple version: your migraine medication Nurtec (rimegepant) is cleared from your body by a liver enzyme called CYP3A4. Lab studies suggest a compound in Rabdosia rubescens (oridonin) may speed up that enzyme. If that happens in real life, your body could break down rimegepant faster, leaving less of it around to do its job. The practical worry is reduced benefit, meaning your migraine might not respond as well or your prevention dose may not hold up.

I want to be honest about the strength of this, though. It comes only from test-tube and animal research, and it has never actually been reported in people. So I wouldn't panic. Rimegepant is not a prodrug, so the direction here is weaker effect, not more side effects. Before combining them, just run it by your pharmacist or doctor.

What These Ratings Mean

Every interaction above is graded three ways — here’s the key to each label.

Severity how clinically significant the interaction is
Major

Clinically significant. Avoid the combination, or use only under close professional supervision.

Moderate

Usually worth avoiding or managing — may need monitoring, a dose change, or timing apart.

Minor

Generally not serious. Be aware of it and mention it to your pharmacist.

Likelihood how strong the evidence is that the interaction actually happens in people
  • LikelyWell-controlled human studies have demonstrated the likely existence of this interaction.
  • ProbableInteraction has not been documented in well-controlled studies, however, the interaction has been demonstrated in some small human studies or in controlled animal studies in conjunction with multiple case reports.
  • PossibleInteraction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
  • UnlikelyInteraction has been demonstrated in animal or in lab research but has been shown not to occur in humans.
Evidence the quality of the studies behind the rating (graded A–D)
  • AHigh-quality meta-analysis (quantitative systematic review).
  • BCase-control study.
  • CConsensus.
  • DAnecdotal evidence.

These ratings are for information only. Never start, stop, or otherwise change your therapy before speaking with your provider.

From our pharmacist’s overview on the Rimegepant page

“Rimegepant (Nurtec ODT), the dissolving migraine tablet, calls for real care alongside supplements, and three entries deserve your attention before anything else. Grapefruit can block the CYP3A4 enzyme that clears rimegepant, raising its blood levels and the chance of side effects.”
