Romidepsin falls under P-GLYCOPROTEIN SUBSTRATES — the group Rabdosia Rubescens interacts with.

Rabdosia rubescens might increase levels of p-glycoprotein substrates. In vitro research shows that Rabdosia rubescens extract inhibits p-glycoprotein levels and activity. Drugs that might be affected include some chemotherapeutic agents (doxorubicin, etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the gist for Romidepsin (Istodax): your body uses a kind of cellular bouncer called P-glycoprotein to pump certain drugs back out and keep their levels in check. Lab studies suggest Rabdosia rubescens can block that pump. If it slows the pump that normally helps move Romidepsin along, more of the drug could linger in your system, which may mean a stronger effect and more side effects like nausea, fatigue, low blood counts, or heart rhythm changes.

Now, the honest part: this is only test-tube evidence, not something proven in people, so the real-world risk is uncertain. Still, Romidepsin is a serious chemo drug, so please run any supplement past your oncologist or pharmacist first before combining them.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Romidepsin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's the concern with pairing this herb and Romidepsin (Istodax). Your body relies on a liver enzyme called CYP3A4 to break down romidepsin. Lab and animal research suggests oridonin, a compound in Rabdosia rubescens, may speed up that enzyme. If it does, your body could clear romidepsin faster than intended, which might mean less of it reaching your cancer and a weaker effect against your lymphoma.

I want to be honest about the strength of this: it has only been seen in test tubes and animals, never confirmed in people, so it's more of a theoretical caution than a proven problem. Still, romidepsin is a serious cancer treatment where you don't want surprises. Please run any supplement past your oncologist or pharmacist before starting it.

What Is Romidepsin Injection Used For?

Brand name: Istodax

Romidepsin injection is approved to treat cutaneous T-cell lymphoma (CTCL) — a cancer that starts in white blood cells and primarily affects the skin — in adult patients who have already received at least one prior systemic (whole-body) therapy.

It is given intravenously (through a vein) in a clinical or infusion-center setting. It is not approved or recommended for peripheral T-cell lymphoma.
