Romidepsin falls under Qt Interval-Prolonging Drugs — the group American Hellebore interacts with.

American hellebore, also known as Veratrum viride, contains alkaloids that can potentially prolong the QT interval on an electrocardiogram (ECG). This effect is similar to that of certain medications known as QT-prolonging drugs, which can increase the risk of a serious heart rhythm disorder called torsades de pointes (source: https://www.ncbi.nlm.nih.gov/pmc/articles/PMC4728534/). Combining American hellebore with other drugs that cause QT-prolongation can increase the risk of adverse effects.

Here's the concern with these two together. Romidepsin (Istodax) is a chemo drug that can stretch out the heart's electrical reset time, something we call QT prolongation. American Hellebore carries its own heart-rhythm risks from its toxic alkaloids, which can also affect the QT interval and badly slow heart rate and blood pressure. Stacking them could raise the chance of a dangerous rhythm called torsades de pointes.

This isn't a prodrug situation, so the worry is simply two QT-stretching effects piling up. Honestly, American Hellebore is poisonous and isn't a safe supplement to begin with. Please don't combine it with Romidepsin, and talk with your oncology team or pharmacist before using any product containing it.

What Is Romidepsin Injection Used For?

Brand name: Istodax

Romidepsin injection is approved to treat cutaneous T-cell lymphoma (CTCL) — a cancer that starts in white blood cells and primarily affects the skin — in adult patients who have already received at least one prior systemic (whole-body) therapy.

It is given intravenously (through a vein) in a clinical or infusion-center setting. It is not approved or recommended for peripheral T-cell lymphoma.
