Sofosbuvir, Velpatasvir, Voxilaprevir falls under P-GLYCOPROTEIN SUBSTRATES — the group Rabdosia Rubescens interacts with.

Rabdosia rubescens might increase levels of p-glycoprotein substrates. In vitro research shows that Rabdosia rubescens extract inhibits p-glycoprotein levels and activity. Drugs that might be affected include some chemotherapeutic agents (doxorubicin, etoposide, paclitaxel, vinblastine, vincristine, vindesine), antifungals (ketoconazole, itraconazole), protease inhibitors (amprenavir, indinavir, nelfinavir, saquinavir), H2 antagonists (cimetidine, ranitidine), some calcium channel blockers (diltiazem, verapamil), digoxin, corticosteroids, erythromycin, cisapride (Propulsid), fexofenadine (Allegra), cyclosporine, loperamide (Imodium), quinidine, and others.

Here's the short version: your Vosevi (sofosbuvir, velpatasvir, voxilaprevir) is moved around your body in part by a tiny pump called P-glycoprotein. Lab studies suggest Rabdosia rubescens may block that pump. If that happened in people, more of the medication could stay in your system, which in theory could mean slightly higher drug levels and a bit more chance of side effects like headache, nausea, tiredness, or diarrhea.

I want to be honest about the evidence though. This is based only on test-tube research, not real people, so we genuinely don't know if it matters in your body. Since Vosevi is a hard-won hepatitis C cure you don't want to disturb, I'd skip this herb during treatment unless your pharmacist or doctor okays it.

CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES

Sofosbuvir, Velpatasvir, Voxilaprevir also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

In vitro research shows that oridonin, a constituent of Rabdosia rubescens, induces cytochrome P450 3A4 (CYP3A4) enzyme. Also, in an animal model, oridonin induces a gene homologous to CYP3A4. So far, this interaction has not been reported in humans. However, watch for a decrease in the levels of drugs metabolized by CYP3A4 in patients taking Rabdosia rubescens. Some drugs metabolized by CYP3A4 that might be affected are some benzodiazepines such as alprazolam (Xanax), diazepam (Valium), midazolam (Versed), triazolam (Halcion); calcium channel blockers such as amlodipine (Norvasc), diltiazem (Cardizem, Dilacor, others), felodipine (Plendil), verapamil (Calan, Verelan, others); cyclosporine (Neoral, Sandimmune); some HIV antivirals such as indinavir (Crixivan), ritonavir (Norvir), saquinavir (Fortovase, Invirase); some HMG CoA reductase inhibitors such as atorvastatin (Lipitor), lovastatin (Mevacor); some macrolide antibiotics such as erythromycin and clarithromycin (Biaxin); quinidine; and many others.

Here's the concern with pairing this herb and your hepatitis C treatment (Vosevi). Lab research suggests oridonin, a compound in Rabdosia rubescens, may speed up a liver enzyme called CYP3A4. Velpatasvir and voxilaprevir, two of the three active ingredients in Vosevi, are partly handled by that enzyme. If the herb revs it up, your body could clear these medicines faster, leaving lower levels in your blood.

Why that matters: Vosevi is a curative course, and you need full drug levels to fully knock out the virus. Lower levels could mean the treatment doesn't work as well. The good news is this is only test-tube and animal data, not seen in real people yet. Still, I'd skip the herb during treatment and check with your pharmacist or doctor first.

What Is Sofosbuvir, Velpatasvir, and Voxilaprevir Used For?

Brand name: Vosevi

Vosevi is used to treat chronic hepatitis C virus (HCV) infection in adults without cirrhosis (liver scarring) or with compensated cirrhosis (mild liver scarring, Child-Pugh A). It is specifically for people who have already tried an HCV treatment that did not fully work.

Vosevi is approved for adults with HCV genotypes 1, 2, 3, 4, 5, or 6 who were previously treated with a regimen containing an NS5A inhibitor. It is also approved for adults with genotype 1a or 3 who were previously treated with sofosbuvir without an NS5A inhibitor. Note: Vosevi has not been shown to offer additional benefit over sofosbuvir/velpatasvir for adults with genotype 1b, 2, 4, 5, or 6 who received sofosbuvir without an NS5A inhibitor.
