These two interact in 2 distinct ways. Each one is explained separately below.
Why Are There 2 Interactions?
Rauwolscine interacts with whole categories of medication. Dextromethorphan HBr, Phenylephrine HCl falls into 2 of those categories, so it can interact with Rauwolscine in 2 different ways. We’ve listed each one below — your overall risk is driven by the most serious of them (Moderate).
CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES
1 of 2Why this applies to Dextromethorphan HBr, Phenylephrine HCl
Dextromethorphan HBr, Phenylephrine HCl falls under CYTOCHROME P450 2D6 (CYP2D6) SUBSTRATES — the group Rauwolscine interacts with.
Rauwolscine is structurally related to yohimbine. In vitro research shows that yohimbine inhibits CYP2D6 enzyme activity.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
Here's the gist with this combo: your body uses an enzyme called CYP2D6 to clear out the dextromethorphan (the cough suppressant) in Vicks Formula 44D. Rauwolscine is a cousin of yohimbine, and lab studies suggest yohimbine can slow that same enzyme down. If that holds true, dextromethorphan could build up a bit more than usual, which might mean more drowsiness, dizziness, or in higher amounts that floaty, confused feeling people get from too much of it.
I want to be honest though: this is theoretical, based on test-tube data rather than real people, so it may never amount to much. Still, since rauwolscine itself raises heart rate and blood pressure (and so can the phenylephrine here), it's worth a quick chat with your pharmacist before pairing them.

STIMULANT DRUGS
2 of 2Dextromethorphan HBr, Phenylephrine HCl also falls under STIMULANT DRUGS.
Rauwolscine has demonstrated stimulant effects in animal research.
Interaction has been documented in animal or in lab research, or the interaction has been documented in humans but is limited to case reports or conflicting clinical research exists.
The ingredient I want to focus on here is the phenylephrine in this cough and congestion product. Phenylephrine is a decongestant that works by tightening up blood vessels, which can nudge your blood pressure and heart rate upward. Rauwolscine is a yohimbine-type stimulant that may boost your body's own norepinephrine, so pairing the two could stack stimulant effects on top of each other. You might notice a faster or pounding heartbeat, jitteriness, a rise in blood pressure, headache, or trouble sleeping.
That said, this concern is mostly theoretical and comes from animal studies, not strong human data. Still, if you have heart issues, high blood pressure, or anxiety, I'd run this combination by your pharmacist or doctor before mixing them.
