Theoretically, taking rauwolscine with seizure threshold lowering drugs might increase the risk of adverse convulsant effects.
In animal research, intraperitoneal rauwolscine lowered the seizure threshold level of the drug metrazol.

Here's the gist: both of these can nudge your brain toward seizures, and stacking them may add up. Hydromorphone is a strong opioid painkiller, and at higher doses (or in people sensitive to it) opioids can lower the seizure threshold, meaning a seizure becomes a little more likely. Rauwolscine, a stimulant found in some fat-burner and pre-workout products, seems to do something similar based on the evidence we have.

Keep this in perspective, though. The concern here comes from animal lab research, not from real people, so it's mostly theoretical. Even so, mixing a stimulant with a potent opioid isn't something I'd take lightly. Watch for jitteriness, a racing heart, confusion, or any twitching or seizure activity, and please run this combo by your doctor or pharmacist before using them together.

Rauwolscine (alpha-yohimbine) is a stimulant alkaloid closely related to yohimbine that is marketed mainly in fat-burning and pre-workout supplements. Human evidence for its benefits is very limited, and it can raise heart rate and blood pressure and cause anxiety, so it should be used with caution and is not recommended for people with heart conditions, anxiety, or during pregnancy or breastfeeding.

What Is Hydromorphone Used For?

Brand names: Dilaudid, Hydromorph Contin, Palladone

Hydromorphone is used to manage pain severe enough to require an opioid analgesic — when non-opioid medicines or other options have not worked or are not expected to work well enough. It is available in several forms: oral tablets, oral solution, rectal suppositories, and injectable formulations given into a vein, muscle, or under the skin.

Hydromorphone hydrochloride extended-release tablets are specifically for severe, persistent pain that requires around-the-clock opioid treatment for an extended period, and are only for opioid-tolerant patients — they are not for as-needed use. The high-potency injectable formulation is also reserved for opioid-tolerant patients who need higher opioid doses.
