Pramlintide falls under ANTIDIABETES DRUGS — the group Reishi Mushroom interacts with.

Theoretically, reishi mushroom might have additive effects with antidiabetes drugs.
Animal research suggests that reishi mushroom decreases blood sugar. However, in patients with type 2 diabetes, taking reishi mushroom does not reduce fasting glucose levels, and its effects on glycated hemoglobin are inconsistent.

Here's the short version: reishi may, in theory, nudge your blood sugar down a little, and pramlintide (Symlin) already lowers blood sugar by slowing how fast food leaves your stomach. Stack those together and there's a chance the two effects add up, which could push you into a low (hypoglycemia). With pramlintide you'd usually feel that as shakiness, sweating, dizziness, hunger, or confusion, especially since it's taken right alongside your mealtime insulin.

Now, the honest part: this concern comes mostly from animal and lab studies. In people with type 2 diabetes, reishi hasn't reliably lowered fasting sugar, so this may not amount to much in real life. Still, don't start reishi without checking with your pharmacist or doctor first, and keep an eye on your glucose readings if you do.

SymlinPen (pramlintide) is used as an add-on treatment for adults with type 1 or type 2 diabetes who already use mealtime insulin but still haven't reached their target blood sugar levels — even with optimal insulin therapy.

It is not a replacement for insulin. It works alongside insulin, given right before major meals, to reduce the spike in blood sugar that happens after eating.
