Abiraterone Acetate falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES — the group Schisandra interacts with.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the practical takeaway if you take abiraterone (Yonsa) for prostate cancer and are eyeing schisandra. Your body relies on a liver enzyme called CYP3A4 to help clear abiraterone. Schisandra tends to slow that enzyme down, so the drug can build up higher than expected in your bloodstream.

More abiraterone in your system can mean stronger side effects, like high blood pressure, fluid buildup and swelling, low potassium, fatigue, or liver enzyme changes. Now, this concern comes mostly from lab and animal work rather than direct studies with abiraterone, so it's more theoretical than proven. Still, given that this is a cancer medicine where dosing matters, I'd run schisandra past your oncology team or pharmacist before adding it.

About Abiraterone

What Is Abiraterone Used For?

Brand names: Zytiga, Yonsa

Abiraterone acetate tablets are used to treat two types of advanced prostate cancer: metastatic castration-resistant prostate cancer (CRPC — cancer that has spread and keeps growing despite low testosterone levels) and metastatic high-risk castration-sensitive prostate cancer (CSPC — cancer that has spread but still responds to hormone-lowering treatment). It is always taken together with a corticosteroid — most products pair with prednisone, while Yonsa is specifically indicated with methylprednisolone.

All products — including Abiraterone, Abiraterone Acetate, Abirtega, Yonsa, and Zytiga — are taken by mouth as a tablet. Yonsa is approved only for metastatic CRPC, while the other products (Abiraterone, Abiraterone Acetate, Abirtega, and Zytiga) are approved for both metastatic CRPC and metastatic high-risk CSPC.
