Why this applies to Flibanserin

Flibanserin falls under CYTOCHROME P450 2C19 (CYP2C19) SUBSTRATES — the group Schisandra interacts with.

Theoretically, schisandra might increase the levels and clinical effects of CYP2C19 substrates.
In vitro research shows that schisandra inhibits CYP2C19, and animal research shows that schisandra increases the concentration of voriconazole, a CYP2C19 substrate. Theoretically, schisandra may also inhibit the metabolism of other CYP2C19 substrates. This effect has not been reported in humans.

Here's the gist for Addyi (flibanserin): your liver uses several enzymes to clear it, and one of them is CYP2C19. In lab and animal studies, schisandra appears to slow that enzyme down. If that happened in people, flibanserin could build up in your system and hit harder than expected. With this drug, that usually means more dizziness, drowsiness, nausea, fatigue, and a real risk of your blood pressure dropping or even fainting, especially since flibanserin already carries those warnings.

I want to be honest about the evidence though. This effect has only been seen in test tubes and animals, not in humans, so it's theoretical at this point. Flibanserin is not a prodrug, so the concern is too much, not too little. Still, since this drug already has serious blood pressure cautions, please check with your pharmacist or doctor before combining them.

Flibanserin also falls under CYTOCHROME P450 3A4 (CYP3A4) SUBSTRATES.

Schisandra can increase the levels and clinical effects of drugs metabolized by CYP3A4.
Most clinical and laboratory research shows that schisandra, administered either as a single dose or up to twice daily for 14 days, inhibits CYP3A4 and increases the concentration of CYP3A4 substrates such as cyclophosphamide, midazolam, tacrolimus, and talinolol. Although one in vitro and animal study shows that schisandra may induce CYP3A4 metabolism, this effect appears to be overpowered by schisandra's CYP3A4 inhibitory activity and has not been reported in humans.

Here's the thing to know about pairing Schisandra with flibanserin (Addyi). Your body relies on a liver enzyme called CYP3A4 to break flibanserin down and clear it out. Schisandra appears to slow that same enzyme, so flibanserin can build up higher than expected and stick around longer. This isn't a prodrug, so more drug in your system means stronger effects.

With flibanserin specifically, the main worry is low blood pressure, fainting, dizziness, and drowsiness, which can be dangerous if you stand up quickly or are driving. Honestly, the human evidence here is limited and mostly comes from lab and animal work, so I don't want to alarm you. Still, please check with your pharmacist or doctor before combining the two.

What Is Flibanserin Used For?

Brand name: Addyi

Addyi is approved to treat acquired, generalized hypoactive sexual desire disorder (HSDD) — a condition where low sexual desire causes significant distress or relationship difficulty — in women under 65. "Acquired" means the low desire developed after a period of normal desire; "generalized" means it happens regardless of the situation or partner.

Addyi is not intended for use in men, and it is not a sexual performance enhancer. It is also not meant for HSDD that is caused by a medical condition, a psychiatric condition, relationship problems, or another medication.
